MovingOn Guide 2014-2015
The MovingOn Transition Planning Guide is compiled and produced by the National Disability Coordination Officer (NDCO Hunter-Central Coast) in collaboration with the Hunter and Central Coast School Leaver Committees.
The transition from school to adult life for young people with disability or an ongoing medical condition is recognised as a critical planning time. Equally difficult is the access to comprehensive information for people with disability in the community seeking to move forward. This extensive directory links into local support personnel, student resources, alternative learning centres, avenues to pursue apprenticeships and traineeships, post-school programs and support services for employment.
